At Stoke House we offer full time residential care to older people who find managing in their own homes increasingly difficult. We have a dedicated activity team whose focus is to ensure that all residents lives have meaning and purpose, from helping out in our tuck shop to caring for our pets. We have strong links with the local community and we arrange bi-monthly trips to surrounding areas of interest.
To ensure that the care we provide matches the individual needs and preferences of residents we encourage both people living with us and their loved ones to be actively involved in creating comprehensive care plans, with dignity and respect at the centre of all we do.
Our single occupancy bedrooms can be personalised to suit the individuals needs and tastes.
We have fully trained registered nurses on duty 24/7 who are trained to support people with a variety of health needs. Our aligned GP Practice is the Netherfield Medical Centre. Their Practioners visit the home fortnightly and also provide a call out service as and when needed.
The nursing team is led by a registered home manager who is also a registered nurse. All care and treatment plans are written and updated by the nursing team to ensure that care delivery remains tailored to each resident’s individual needs, and we encourage participation from both residents and relatives. All activities on offer can be adjusted to include people of all abilities and when external visits are arranged we use specialised transport with full wheelchair access.
We have a dedicated team of experienced and enthusiastic care support workers who are trained in supporting people living with Dementia. This includes having an understanding of a person’s behaviours and how they express themselves, and staff take a flexible approach when supporting a person living with dementia.
Our Environment has been carefully thought out to provide stimulation, safety and security and give a home from home feel.
We have close links with the dementia outreach team who are on hand to provide additional advice and support, and through which we can access a MHSOP (Mental Health Services for Older People) specialist consultant and community psychiatric nurses to ensure that the care we provide is best suited to the individual living with dementia.
At Stoke House we can provide the care and support of a loved one who is suffering from a terminal or incurable condition. Our aim is to ensure that people are cared for in clam and comfortable surrounds by experienced staff who can guide and support you each step of the way.
We are able to offer both residential and nursing care for short stay periods. This can range from a few days to several weeks, subject to an initial assessment (Min stay 5 nights, dependent on availability).
Day Care and Night Care
We are able to create tailored packages to meet individual needs. This includes day care where our staff will carry out all care needs and the individual will have access to our facilities and any activities planned for that day. This can also include support with bathing and shaving. We have a hairdresser visit each week and our activity staff can provide nail care, hand massages and other pampering sessions.
If you have a relative who requires support at night we can offer them a private bedroom for overnight care. Again, this package can be tailored to meet individual needs.
